The shoulder blade, or scapula, plays an essential role in nearly every movement of the arm. It helps stabilize the shoulder joint and works with surrounding muscles to keep the upper arm moving smoothly.
When the muscles that control the scapula become weak, tight, injured, or imbalanced, the shoulder blade may no longer rest or move normally. This condition is known as scapular dyskinesis and can lead to pain, weakness, limited motion, and difficulty with overhead activities.
What is Scapular Dyskinesis?
Scapular dyskinesis refers to an alteration in the normal position or movement of the shoulder blade.
As the arm moves, the scapula should rotate and shift in a coordinated way to maintain proper alignment of the shoulder’s ball-and-socket joint. When this motion becomes abnormal, the shoulder may feel weak or unstable and may become more vulnerable to injury.
In some patients, the inner border of the shoulder blade becomes more prominent, creating what is commonly called a “winged” scapula.
Common Symptoms
Patients with scapular dyskinesis may experience:
- Pain or tenderness around the shoulder blade
- Weakness or fatigue in the affected arm
- Difficulty with repetitive or overhead activities
- Limited range of motion
- Inability to comfortably raise the arm above shoulder height
- Crunching, snapping, or popping with shoulder movement
- Visible “winging” or protrusion of the shoulder blade
- A drooping or forward-tilted posture on the affected side
Some patients describe the arm as feeling “tired” or “dead” during vigorous activity.
Diagnosis
Diagnosis usually begins with a detailed physical examination.
A healthcare provider may observe the shoulder blades from behind while the patient moves the arms up and down or pushes against a wall. This can help reveal abnormal movement patterns or muscle weakness.
Additional evaluation may include:
- Strength testing of the shoulder and scapular muscles
- Range-of-motion assessment
- Scapular assistance testing
- Scapular retraction testing
Imaging is not always necessary, but X-rays, CT scans, or MRI may be ordered if another shoulder injury or bone abnormality is suspected.
If nerve damage is a concern, nerve conduction testing may also be recommended.
Treatment Options
Physical Therapy
Most cases improve with nonsurgical treatment, and physical therapy is often the primary treatment.
Therapy typically focuses on:
- Strengthening the muscles that stabilize and move the shoulder blade
- Stretching tight muscles that limit normal motion
- Improving shoulder mechanics
- Restoring proper scapular position during arm movement
Correcting muscle imbalance and movement patterns can help reduce pain and improve function.
Additional Conservative Care
Other strategies may include:
- Anti-inflammatory medications when appropriate
- Heat therapy for tight shoulder muscles
- Posture correction
- Activity modification
- A balanced strengthening and flexibility program
Good posture is especially important because prolonged forward-rounded shoulder positioning may contribute to abnormal scapular mechanics.
Maintaining Healthy Shoulder Mechanics
Once normal motion and strength have been restored, ongoing conditioning may help prevent symptoms from returning.
Patients may benefit from:
- Regular shoulder and upper-back strengthening
- Stretching tight chest and shoulder muscles
- Maintaining good posture
- Avoiding repetitive overuse
- Balancing pushing and pulling exercises
- Following a maintenance exercise program recommended by a healthcare provider or physical therapist
This can be especially important for patients whose jobs, sports, or hobbies require frequent overhead or repetitive arm movements.
When to Seek Medical Evaluation
Patients should consider further evaluation if they experience:
- Persistent pain around the shoulder blade
- Visible shoulder blade “winging”
- Weakness or fatigue during arm movement
- Difficulty raising the arm overhead
- Repeated popping or snapping around the shoulder
- Symptoms that interfere with work, exercise, or daily activities
Early evaluation can help identify the cause of abnormal scapular movement and guide appropriate treatment.
